Behavior Supports

Supported Living Foundations is excited to soon offer Behavioral Support Services in Oregon! Behavior Supports are Medicaid-authorized interventions designed to assist individuals with intellectual or developmental disabilities (I/DD), mental health conditions, or challenging behaviors. The goal is to help each person achieve greater independence, stability, and quality of life within their homes and communities.

These services typically include:

  • Functional Assessments: Identifying the underlying causes of behaviors and determining effective support strategies.

  • Behavior Support Planning: Developing individualized plans that promote positive behaviors and reduce challenging ones.

  • Staff Training: Equipping caregivers and support staff with the skills to implement behavior plans effectively.

  • Ongoing Monitoring: Ensuring that interventions remain effective and are adjusted as needed over time.

Behavioral supports are essential to ensure individuals receive person-centered, evidence-based care that fosters inclusion and long-term success.

Day Support Activities (DSA)

Supported Living Foundations is preparing to offer Community-Based Day Support Activities (DSA)!  DSA offers structured opportunities for adults with intellectual or developmental disabilities (I/DD) to stay active and connect with others in their communities.  Unlike traditional center-based programs, community-based DSA services take place in real-world settings—such as parks, libraries, volunteer sites, and local businesses.

Individuals may receive individualized one-on-one support, or join small or large groups.  Through DSA, individuals gain opportunities to participate fully in their communities, develop meaningful connections, and enhance their overall quality of life.

Day Supports emphasize:

  • Communication and social skills – building confidence and positive relationships.

  • Community engagement – fostering inclusion and valued roles in everyday life.

  • Skill development – practicing daily living, volunteering, and educational activities.

  • Behavioral and emotional support – encouraging self-expression and independence.
