About SLF
Formed in 2021, Supported Living Foundations was established with a mission to promote independence and self-determination within the adult Intellectual and Developmental Disabilities (I/DD) community through the supported living model.

Over time, our organization has expanded to provide a broader range of services, including Training & Professional Development, Day Support Activities, and Behavior Supports.

At Supported Living Foundations, we are committed to empowering individuals by delivering high-quality, person-centered supports that encourage growth, inclusion, and meaningful community engagement.
