Our Supported Living Program is designed to promote independence, empowerment, and progress through individualized support that focuses on each person’s unique goals and needs. Individuals in the Supported Living program live in their own homes—or with their families—and receive tailored support to help them thrive. Our focus is always on health, safety, and personal growth, ensuring each person can maintain a healthy, independent, and meaningful lifestyle.

Program Highlights:

Practical Skills for Independent Living: Building daily living skills such as cooking, cleaning, budgeting, and personal care.

Informed Decision-Making: Encouraging individuals to make choices that reflect their values, preferences, and goals.

Social Skills for Healthy Relationships: Supporting communication, community involvement, and positive relationship-building.​​​​

Supports may Include:

Financial Budgeting: Support with financial planning, money management, and bill paying to promote independence and stability.

Meal Preparation: Assistance with meal planning, grocery shopping, and food preparation. Support may also include help applying for SNAP benefits.

Medical Management: Help with scheduling and attending medical appointments. Medication management is available for individuals who need additional support.

House Cleaning: Assistance with household cleaning and skill-building to maintain a safe and comfortable living environment.

Transportation: Transportation provided for medical appointments and other essential or community events.

Goal Building: Support with individualized goal planning, progress tracking, and developing routines that encourage personal growth.

Social Life: Opportunities to participate in weekly and annual events organized by Supported Living Foundations, as well as activities tailored to individual interests and preferences.

Crisis Support: 24/7 emergency contact and assistance to ensure safety and peace of mind
